Abstract

Introduction: Cotard syndrome is a rare neuropsychiatric condition where the patient denies the existence of parts of his body or his own existence.


Objective: Describe the psychopathological alterations in a patient with a diagnosis of intellectual disability and Cotard syndrome.


Clinical case: Male, 18 years old, low academic performance; It began at the age of 9 with refusal of food, theft of objects and lack of hygiene. At the time of his admission, he reports that he is constantly pursued by men who want to kill him again (delusional ideas of harm) and coenestopathic delusions. During admission, mental retardation is diagnosed and nihilistic delusions are reinforced. He is subjected to electroconvulsive therapy without results. After several weeks of admission, remission is achieved
with the combination of clozapine, lithium carbonate and levomepromazine.


Conclusions: Cotard syndrome in association with intellectual disability is a rare diagnosis of exclusion
