Introduction: contemporary medical education is called upon to train competent professionals, capable of critically interpreting the complexity of clinical phenomena. Contrastive reasoning, as a fundamental resource, enhances diagnostic judgment.
Objective: to assess the impact of contrastive reasoning as a logical foundation of clinical learning in contemporary medical education.
Method: a narrative review of the scientific literature was conducted across different databases, using an algorithm to identify relevant sources. These were selected after applying rigorous inclusion and exclusion criteria. The studies were evaluated in terms of timeliness, methodological quality, and thematic relevance, and were coherently integrated into the final synthesis of the review.
Results: contrastive reasoning constitutes an essential component of clinical reasoning, as it fosters the formulation and confrontation of diagnostic hypotheses. An insufficient application of critical strategies was identified in traditional methodologies focused on memorization, which limits students’ ability to face uncertain clinical scenarios. Furthermore, it was demonstrated that contrastive reasoning strengthens the construction of differential diagnoses, promotes systematic doubt regarding first impressions, and contributes to the prevention of cognitive biases.
Conclusions: contrastive reasoning, characterized by the binary relationship of opposites, represents an indispensable logical basis for medical training. Its systematic application in teaching enhances analytical capacity, the elaboration of differential diagnoses, and evidence-based clinical decision-making, consolidating a critical and safe learning process.
